What it is, How it works

Hair analysis is now recognized as a valuable method for detecting drug and alcohol consumption. Hair captures a long-term record of substances by embedding biomarkers within the growing strand's fibers. When obtained near the scalp, it offers a detection window of up to three months for alcohol and drugs. Hair is easy to collect, relatively tamper-proof, and straightforward to transport.

A sample measuring 1.5 inches, consisting of around 200 hair strands (similar in thickness to a #2 pencil) near the scalp yields 100mg, the preferred amount for testing. For EtG enhancements or tests exceeding 10 panels, a 150mg sample is advised. It’s best to measure the sample on a jeweler’s scale. In cases where scalp hair is not an option, a comparable quantity of body hair is acceptable. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is entered into the lab's system. This step involves ensuring the sample is correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody.

Screening offers a preliminary check for drug use. Although Screening is an economical approach for eliminating drug usage in most samples, a positive result requires confirmation for judicial acceptance. Presumptively positive samples in Screening need further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for Extraction. In this phase, drugs are separated from the hair at much lower concentrations than those found in alternative methods (like urine or saliva), hence hair testing proves to be the most challenging to execute.

To confirm any positive Screening outcome, GC/MS, GC/MS/MS, or LC/MS/MS methods are used. All presumptive positives are cleansed before confirmation, if necessary. The complete lab procedure from Accessioning to Confirmation complies with CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
