What it is, How it works

When sampled near the scalp, hair can offer a detection window for alcohol and other drugs extending up to around 3 months. Hair is straightforward to obtain, relatively resistant to tampering, and simple to transport.

A 1.5-inch segment comprising about 200 hairs (comparable to the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, the optimal sample size for analysis and verification. For EtG, additional assessments, and/or panels exceeding 10 tests, 150mg of the specimen is suggested. We advise utilizing a jeweler’s scale for weighing the specimen. If head hair is unavailable, an equivalent quantity of body hair can be collected. When speaking of head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedures for processing a drug test outcome encompass four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the step where a sample is initially entered into a laboratory’s system. This involves confirming that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry absent from an electronic chain of custody framework.

Screening includes an initial swift examination for drugs of abuse. Although Screening is a cost-effective approach to negate drug usage in most samples, a positive screen must be validated to be permissible in legal proceedings. Any samples preliminarily positive in Screening require secondary confirmation.

Should a sample screen positive in the Screening phase, additional hair is extracted from the initial specimen and prepped for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Verification of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All preliminarily positive samples are washed before confirmation as deemed necessary. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards certification.

The manuscripts in this section presented novel approaches, such as multianalyte methods and on-line preparation which can enable rapid analysis and improve thorough forensic investigations.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
