What it is, How it works

Hair captures a long-term record of alcohol and other substances by embedding biomarkers within the fibers of the growing hair strand. When collected near the scalp, hair can provide an approximate detection period of up to 3 months for alcohol and other substances. Hair collection is straightforward, relatively hard to tamper with, and easy to transport.

An ideal sample for screening and confirmation is a 1.5-inch section consisting of roughly 200 hair strands (similar in size to a #2 pencil), taken closest to the scalp, yielding 100mg. For EtG, additional assessments, and/or tests exceeding 10 panels, a specimen weighing 150mg is advised. The specimen should be weighed on a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair can be collected. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

Accessioning, Screening, Extraction, and Confirmation are the four crucial steps in the laboratory processing of a drug test result.

Accessioning entails the initial incorporation of a sample into a lab’s system. This process includes verifying if the sample was appropriately sealed and sent, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data entry that may not be covered by an electronic chain of custody system.

Screening serves as an initial rapid check for drugs. Although Screening is a cost-efficient way to rule out drug use for most samples, a positive screen must be verified for court admissibility. Any samples with a presumptive positive result during Screening need a secondary confirmation.

If a sample shows presumptive positivity in the Screening phase, additional hair is withdrawn from the initial specimen for Extraction preparation. Drugs are extracted at a substantially lower concentration from hair compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation, if needed. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
