A 1.5-inch segment of roughly 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and verification. For EtG, supplementary tests, and tests exceeding 10 panels, a 150mg sample is advisable. The specimen should be weighed using a jeweler’s scale for precision. If scalp hair is not available, an equivalent quantity of body hair can serve as a substitute. In terms of hair, "head hair" pertains solely to scalp hair, while "body hair" includes any other varieties (facial, axillary, and so forth).

Process Overview

The essential phases in lab processing of a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the procedure of registering a sample into the laboratory’s records. This process involves ensuring the sample’s seal and shipment were intact, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry lacking from an electronic chain of custody system.

Screening involves a preliminary inspection for drugs commonly abused. While Screening provides an economical approach to rule out drug usage in most samples, a positive result in Screening requires confirmation for legal acceptability. Samples that test as presumptive positives through Screening require additional verification.

The entire process, from Accessioning to Verification, adheres to the standards set by both the CAP (College of American Pathologists) Hair designation and the accrediting ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
