What it is, How it works

Hair testing has emerged as a potent instrument for identifying drug and alcohol consumption. Hair captures a historical record of alcohol and drugs by embedding biomarkers in the growing hair strand's fibers. When obtained close to the scalp, hair can offer up to a roughly 3-month window of detection for alcohol and other drugs. Hair is easy to collect, relatively resistant to tampering, and simple to send.

A 1.5-inch segment comprising around 200 strands of hair (resembling the size of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. We advocate weighing the specimen using a jeweler’s scale. If scalp hair is not accessible, a comparable quantity of body hair may be gathered. When talking about head hair, we are exclusively referring to scalp hair. Body hair denotes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s system. This process includes ensuring the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and executing any supplementary data entry not supplied by an electronic chain of custody system.

Screening involves a preliminary rapid evaluation for drugs of abuse. Although Screening is an economical method to exclude drug usage in most samples, a positive screen requires confirmation to be court-admissible. Any samples that are initially positive in Screening necessitate a subsequent confirmation.

If a sample is initially positive during the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening result is undertaken via GC/MS, GC/MS/MS or LC/MS/MS. All initially positive samples undergo washing before confirmation as necessary. The entire laboratory sequence from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection window: Hair drug tests can identify drug usage for up to 90 days, compared to urine tests with a shorter detection duration.
  • Challenging to manipulate: It is extremely challenging to manipulate a hair drug test, enhancing result accuracy.
  • Delivers historical data: It can offer a trend of drug consumption over a timeframe, not solely recent use.

Limitations:

  • Cannot identify recent use: Drugs take approximately 5-7 days to become detectable in hair.
  • Expense: Hair drug tests typically cost more than other drug testing procedures.
  • Results variability: Factors such as hair color and individual variations in hair growth can influence drug metabolite concentration in the hair.

Note: Despite often being termed as "hair follicle tests", the test itself analyzes the hair strand rather than the follicle beneath the scalp.
