Collecting hair is straightforward, relatively resistant to tampering, and simple to transport.

A sample of 1.5 inches with about 200 hair strands (around the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is optimal for testing and validation. For EtG, additional analyses, and/or tests exceeding 10 panels, a specimen of 150mg is advised. It's suggested to measure the specimen using a jeweler’s scale. In cases where scalp hair is absent, a comparable quantity of body hair can be obtained. Mention of head hair pertains solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The principal stages in processing a drug test result at the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary management of a sample within a lab's system. This step involves confirming the sample's seal and proper shipment, allocating a random LAN (Laboratory Accessioning Number), and addressing any further data entry not covered by an electronic chain of custody mechanism.

Screening serves as an initial rapid evaluation for substances of abuse. While Screening is economical for excluding drug use in most samples, any positive findings require confirmation for court acceptance. Samples yielding presumptive positives in Screening necessitate additional confirmation.

For samples deemed presumptively positive during Screening, additional hair is extracted from the initial specimen and readied for Extraction. In this phase, drug substances are extracted at significantly lower concentrations from hair compared to other methods (e.g., urine or oral fluid), making hair drug tests the most challenging to execute.

Confirmation of any positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as applicable. The complete lab procedure from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair designation and adheres to the accreditation of ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
