Process Overview

There are four primary phases in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's entry into the laboratory framework, ensuring the sample's secure sealing and delivery, assigning a LAN (Laboratory Accessioning Number) randomly, and finishing any unincluded data from an electronic chain of custody system.

Screening is a preliminary rapid evaluation for substances of abuse. Although Screening provides a cost-efficient means to dismiss drug use in most samples, a positive screen necessitates confirmation to be legally valid. Any specimens with a presumptive positive in Screening need a follow-up confirmation.

If Screening suggests positivity, more hair is extracted from the original sample and readied for Extraction. In this phase, drugs are isolated from hair at a much lower concentration than in techniques like urine or oral fluid analysis, highlighting the complexity of hair drug screening.

Positive screening confirmations are pursued via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens undergo pre-confirmation washing where needed. The entire lab workflow, from Accessioning to Confirmation, is scrutinized following both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
