In the absence of scalp hair, a comparable amount of body hair can be utilized. References to head hair pertain exclusively to scalp hair, while body hair encompasses other types such as facial or axillary hair.

Process Overview

The laboratory analysis of a drug test outcome is composed of four core steps: Accessioning, Screening, Extraction, and Confirmation.

In Accessioning, a sample is initially processed within the laboratory's framework. This step entails checking the proper sealing and shipment of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any further data entry required that is not covered by an electronic chain of custody system.

Screening involves a preliminary examination for the presence of abusive drugs. This stage is a cost-efficient approach to excluding drug usage in the majority of samples. However, a positive result at this stage must be validated to be admissible legally. Any sample flagged as potentially positive at Screening requires further confirmation.

Should a sample test presumptive positive during Screening, additional hair from the original specimen is used for Extraction. At this point, drugs are separated from the hair at a much lower concentration level than other methods (such as urine or oral fluid), making hair drug testing one of the most intricate methodologies to execute.

Positive Screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS procedures. Any sample that is presumptively positive is cleansed before confirmation as necessary. The entire process from Accessioning through to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and the accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
