For EtG, additional tests, and/or panels exceeding 10, 150mg of sample is recommended. Use a jeweler’s scale to weigh the sample. If scalp hair is not available, equivalent body hair may be used. The term head hair references scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with the initial integration of a sample into a lab’s system. This step verifies proper sealing and shipping of the sample, allocates a random LAN (Laboratory Accessioning Number), and completes any data entry not managed by an electronic chain of custody.

Screening consists of an initial rapid assessment for drugs. While Screening is cost-effective for ruling out drug usage in most samples, a positive result requires confirmation for legal admissibility. Any presumptive positives from Screening must undergo secondary confirmation.

If a sample shows presumptive positivity during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are separated from hair at lower concentrations than with other methods (like urine or oral fluid), making hair drug screening a challenging procedure.

Confirmation of positive screening outcomes is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples are cleansed before confirmation, as necessary. The full laboratory process from Accessioning to Confirmation is evaluated under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
