What it is, How it works

Hair tests have emerged as a robust method for uncovering drug and alcohol consumption. Hair offers a comprehensive timeline of alcohol and drugs by capturing biomarkers within the strands. When collected near the scalp, it offers nearly a 3-month detection period for substances. Hair is easy to gather, hard to tamper with, and simple to transport.

A 1.5-inch section of around 200 hair strands (similar in size to a #2 pencil) closest to the scalp yields 100mg of hair, the perfect amount for analysis and confirmation. For EtG, additional tests, or panels above 10, 150mg is suggested. We advise measuring the specimen on a jeweler’s scale. If head hair isn't available, a similar quantity of body hair can be collected. Head hair refers to scalp hair only. Body hair encompasses other types like facial and axillary hair.

Process Overview

The process of laboratory assessment of a drug test result includes four major stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial entry of a sample into a laboratory’s system. This step involves confirming the sample's sealed and properly shipped status, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any further data entry omitted by an electronic chain of custody system.

Screening entails an initial preliminary test for substances of abuse. Although Screening is a cost-effective method to dismiss drug usage in most samples, a positive screen needs confirmation to be valid in legal settings. Samples with a presumptive positive in Screening must undergo secondary confirmation.

If Screening stage results are presumptively positive, additional hair is taken from the original specimen and prepared for Extraction. In this phase, substances are extracted from hair at lower concentrations compared to other methods (like urine or oral fluid), making hair drug screening the most complex method.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
