For EtG, extended panels, or tests beyond 10 panels, a 150mg specimen is advised. Usage of a jeweler's scale for weighing the sample is suggested. If scalp hair is unavailable, an equivalent quantity of body hair can be gathered. 'Head hair' refers only to scalp hair, while 'body hair' encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab's system, ensuring proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.

Screening provides an initial drug-of-abuse assessment. Though cost-effective for ruling out drug usage in most samples, a positive screen needs confirmation to be legally admissible. Any samples tentatively positive in Screening require secondary confirmation.

If a sample is tentatively positive during Screening, additional hair from the original sample is extracted for further analysis. This step extracts drugs from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as required. The entire laboratory process from Accessioning to Confirmation undergoes review based on the CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.
