What it is, How it works

If scalp hair cannot be obtained, the same amount of body hair can be used. When talking about head hair, we're specifically referring to scalp hair. Body hair covers all other hair types (facial, axillary, etc.).

Process Overview

The key stages in a lab's drug testing procedure include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into the lab's system, validating its seal and shipment, issuing a LAN (Laboratory Accessioning Number), and completing supplementary data entry absent in an electronic custody chain system.

Screening offers an initial assessment for substances. While Screening economically eliminates drug use in most cases, a positive result necessitates confirmation to be valid legally. Samples that flag positive in Screening require further verification.

If Screening indicates a presumptive positive result, additional hair is extracted from the sample for the Extraction phase. Here, the drugs are isolated from hair at significantly lower concentrations than in other mediums (such as urine or saliva), making hair drug analysis the most challenging technique.

Positive screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptive positive sample gets a wash before verification if needed. The complete lab process, from Accessioning to Confirmation, undergoes scrutiny according to both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
