A 1.5-inch section comprising approximately 200 hair strands (comparable to a #2 pencil's thickness) closest to the scalp yields 100mg, which is the optimal amount for screening and confirmation. For EtG additions or tests exceeding ten panels, 150mg is advisable. We suggest using a jeweler's scale for obtaining the specimen's weight. If scalp hair is not obtainable, a similar volume of body hair can be collected. References to head hair indicate scalp hair exclusively. Body hair denotes all other categories (such as facial or axillary hair).

Process Overview

The lab procedure for drug test results consists of four principal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample within a laboratory's ecosystem. This step ensures proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and finishes any required data input not included in an electronic chain of custody system.

Screening offers an initial rapid assessment for substances of abuse. Although Screening is a financially viable method to exclude drug usage in most samples, confirmation is needed for screen-positive outcomes for court admissibility. Samples that are provisionally positive during Screening necessitate secondary confirmation.

If a Screening phase sample is provisionally positive, additional hair from the initial sample is used for Extraction. In this process, drug extraction occurs at a much lesser concentration compared to other methods (such as urine or oral fluid), making hair drug analysis the hardest to execute.

Confirmation of any Screening-positive result employs GC/MS, GC/MS/MS, or LC/MS/MS. All samples with provisional positives undergo pre-confirmation washing as necessary. The entire process spanning from Accessioning to Confirmation is assessed according to both the CAP (College of American Pathologists) Hair certification and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
