A 1.5-inch sample consisting of roughly 200 hair strands (comparable to the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for analysis. For EtG, additional tests, and tests exceeding 10 panels, 150mg is advised. It is advisable to measure the sample using a jeweler’s scale. If scalp hair is unavailable, a comparable amount of body hair may be utilized. References to head hair pertain solely to scalp hair, whereas body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result comprises four fundamental steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s system. This step includes checking for proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not covered by an electronic chain of custody.

Screening offers a preliminary, rapid evaluation for drug abuse. While cost-efficient for ruling out drug use in most samples, a positive result in Screening requires confirmation for legal admissibility. Any samples with presumptive positive results in Screening must undergo a secondary confirmation.

The entire process from Accessioning to Confirmation is evaluated under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
