What it is, How it works

When acquired near the scalp, hair can reveal up to around a three-month detection timeframe for alcohol and other substances. Hair is straightforward to gather, relatively challenging to tamper with, and convenient to ship.

A 1.5-inch portion of roughly 200 strands of hair (similar in size to a #2 pencil) nearest to the scalp will provide 100mg of hair, the optimal sample for analysis and validation. For EtG, add-ons, and/or assessments beyond 10 panels, 150mg of the specimen is advised. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair may be obtained. When referencing head hair, we mean scalp hair exclusively. Body hair denotes all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages involved in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into a lab’s framework. This entails confirming that the sample was sealed and dispatched properly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not provided by an electronic chain of custody system.

Screening embodies an initial swift check for substances of misuse. While Screening is an economical approach to exclude substance usage on the majority of samples, a positive screen requires confirmation to be admissible in legal proceedings. Any samples that presumptively show positive in Screening mandate further confirmation.

If a sample shows presumptive positive in the Screening phase, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, substances are extracted from hair at a significantly lower concentration than in other techniques (such as urine or oral fluid), which is why hair substance screening is the most intricate technique to conduct.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that presumptively show positive are cleansed before confirmation as necessary. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
