For EtG, supplementary tests, and analyses beyond 10 panels, a 150mg sample size is advised. We suggest using a jeweler’s scale for precise measurement. If scalp hair isn't obtainable, body hair can be substituted to achieve an equal mass. When we mention head hair, it specifically pertains to scalp hair, while body hair includes all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed within the laboratory's system. This encompasses ensuring the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing any required data entry not handled by an electronic chain of custody.

Screening is a preliminary evaluation for substances of abuse. While Screening is cost-effective for excluding drug use in most cases, any positive result requires follow-up to be viable in legal settings. Therefore, samples marked as presumptively positive necessitate secondary validation.

If a sample is found presumptively positive during Screening, additional hair is extracted from the initial sample for the Extraction phase. In this process, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine, oral fluid), which is why hair drug screening is one of the most challenging techniques.

Confirmation for any positive screen result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Any samples identified as presumptively positive are cleansed prior to confirmation if needed. The comprehensive lab procedure, from Accessioning to Confirmation, undergoes review in line with both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
