A 1.5-inch section comprising approximately 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp yields 100mg of hair, which is the perfect sample size for screening and verification. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is advised. We suggest using a jeweler's scale to weigh the sample. If scalp hair is lacking, a corresponding quantity of body hair can be used. When discussing head hair, we mean scalp hair, whereas body hair includes all other types (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step, where a sample is integrated into the lab's system. This involves checking that the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary manual data entry not covered by an electronic chain of custody.

Screening entails a preliminary inspection for drug abuse. Although Screening is an economical method to exclude drug use in most samples, a positive screening needs subsequent confirmation for legal acceptance. Any sample that tests as presumptively positive in Screening requires a follow-up confirmation.

If there's a presumptive positive result in the Screening, extra hair is taken from the original sample and readied for Extraction. During this stage, drugs are removed from hair at a much lower concentration compared to other methods (such as urine or saliva), underscoring why hair drug screening is the most intricate method to execute.

Confirmation on any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating potential positives undergo washing if necessary before confirmation. The comprehensive lab procedure from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
