Process Overview

The laboratory process for a drug test result includes four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary processing of a sample within a lab's system. This step ensures that the sample was correctly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry missing from an electronic chain of custody system.

Screening is a preliminary and swift examination for substances of abuse. Although Screening is a cost-efficient method to eliminate drug use in most samples, a positive outcome must be validated to be legally acceptable. Samples with presumptively positive results during Screening necessitate subsequent confirmation.

For samples identified as presumptively positive during Screening, additional hair is retrieved from the main specimen and readied for Extraction, where drugs are extracted from the hair at significantly lower levels than in alternative methods (e.g., urine or oral fluid), making hair drug screening among the most challenging techniques to execute.

Confirmation for any positive results is achieved through methods such as GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as necessary. The entire process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair criteria and certified in line with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
