What it is, How it works

Testing hair has gained recognition as an effective means for uncovering drug and alcohol usage. Hair retains a prolonged history of alcohol and drug use by embedding biomarkers in the strands as they grow. When taken from near the scalp, hair can offer a detection range of up to approximately three months for alcohol and other drugs. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

A hair specimen of 1.5 inches, consisting of around 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal sample size for screening and confirmation purposes. For EtG, add-ons, or analyses beyond 10 panels, 150mg of the sample is advised. We recommend using a jeweler’s scale for weighing the specimen. If head hair is absent, a similar quantity of body hair may be gathered. When mentioning head hair, we mean scalp hair exclusively. Body hair refers to any other hair types (facial, axillary, etc.).

Process Overview

The four primary stages involved in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample into a lab system. This process includes confirming that the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and wrapping up any extra data entry absent from an electronic chain of custody system.

Screening entails a preliminary quick assessment for abusive drugs. While Screening is an economical method to exclude drug use in most samples, a positive screen must be verified to be acceptable in court. Any samples that test presumptively positive during the Screening stage require secondary confirmation.

Should a sample test presumptively positive at the Screening stage, additional hair is taken from the first specimen and readied for Extraction. During this stage, drugs are extracted from hair at a significantly lower concentration than in other methods (for example, urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed pre-confirmation, when necessary. The complete lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
