Securing a 1.5-inch hair sample containing approximately 200 strands (approximately the girth of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimum sample size for both screening and confirmation processes. To include EtG, additional panels, or tests exceeding 10 panels, a sample of 150mg is advised. It's suggested to measure the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair can be collected. "Head hair" references scalp hair exclusively, whereas "body hair" encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory drug test process includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes preliminary processing into the lab's system, including ensuring proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and adding any extra data omitted by an electronic custody chain.

Screening conducts an initial rapid assessment for drugs of abuse. Although Screening provides a cost-effective method to exclude drug use in most samples, any positive screen must undergo confirmation for courtroom admissibility. Thus, all Screening samples that are presumptively positive need subsequent confirmation.

For a presumptive positive sample in the Screening phase, additional hair is extracted from the original sample and prepared for Extraction. At this point, drugs are isolated from the hair at a much finer concentration than in methods like urine or oral fluid testing, rendering hair drug testing the most challenging method to implement.

Confirmation of positive screening outcomes is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positives are cleansed as necessary. The comprehensive lab process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
