A 1.5-inch segment of approximately 200 hair strands (equal to the size of a #2 pencil) nearest to the scalp yields 100mg of hair, ideal for both screening and confirmation. For EtG, add-ons, and/or panels exceeding 10, a 150mg sample is advised. Measuring the sample on a jeweler’s scale is suggested. If scalp hair is absent, an equivalent quantity of body hair can be gathered. Head hair specifically refers to scalp hair, whereas body hair encompasses facial, axillary, and other categories.

Process Overview

The lab processing of a drug test result consists of four key phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially incorporated into a lab’s system. This step involves checking that the sample was adequately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening provides a preliminary assessment for substances of abuse. While cost-effective for ruling out drug use in most samples, a positive screen must be confirmed to be accepted in legal settings. Any samples showing presumptive positives in Screening need subsequent confirmation.

If a sample tests presumptively positive in Screening, additional hair is removed from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at much lower concentration levels than other methods (such as urine or oral fluid), making hair drug screening a particularly challenging method to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples presumed positive are washed before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
