A 1.5-inch portion consisting of around 200 strands of hair (approximately the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal amount for testing and confirmation. For EtG, additional tests, and/or panels exceeding 10 types, a 150mg sample is advisable. We suggest using a jeweler’s scale for weighing the sample. If scalp hair isn't accessible, a comparable volume of body hair can be secured. When mentioning head hair, we are specifically referring to scalp hair. Body hair includes all other hair varieties (such as facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test result involves four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample enters a lab’s system. This phase includes ensuring that the sample was correctly sealed and delivered, allotting a random LAN (Laboratory Accessioning Number), and completing any extra data not supplied by an electronic chain of custody system.

Screening entails an initial assessment for substances of abuse. While Screening is an economical means to dismiss drug use for most samples, a confirmed positive result is necessary for legal acceptance. Any samples returning a presumptive positive in Screening demand a follow-up confirmation.

If a sample shows a presumptive positive in Screening, additional hair is taken from the initial specimen for the Extraction process. In this phase, drugs are withdrawn from hair at a lower concentration compared to other techniques (for instance, urine or oral fluid), making hair drug screening the most intricate testing method.

Confirmation of any positive Screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before Confirmation if necessary. The full lab procedure, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
