For EtG, additional tests, and/or evaluations exceeding 10 panels, collecting 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't available, an equivalent volume of body hair may be used. When discussing head hair, it specifically refers to scalp hair. In contrast, body hair includes other hair forms (facial, armpit, etc.).

Process Overview

The laboratory processing of a drug test result comprises four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the laboratory's system. This phase ensures the sample's proper sealing and shipping, assigns a random Laboratory Accessioning Number (LAN), and finalizes additional data entry not covered by an electronic chain of custody system.

Screening conducts an initial basic evaluation for drugs of misuse. Although Screening is a budget-friendly approach to exclude drug usage in most of the samples, a positive result needs authentication to stand in legal proceedings. Samples showing a presumptively positive result in Screening necessitate further confirmation.

If a specimen shows a presumptive positive outcome in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. At this juncture, drugs are separated from hair at relatively minimal concentrations compared to other methods (like urine or saliva), highlighting the complexity of hair drug screening.

Any positive screening outcome undergoes Confirmation through methods such as GC/MS, GC/MS/MS or LC/MS/MS. All presumptively positive samples undergo washing before confirmation if needed. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
