What it is, How it works

Hair analysis has emerged as a valuable method for identifying drug and alcohol consumption. When collected close to the scalp, hair can offer a detection timeline of up to around 3 months for alcohol and drugs. Hair is straightforward to collect, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch segment consisting of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp provides around 100mg of hair, which is the ideal sample for both screening and confirmation. For EtG, expanded panels, and/or tests exceeding 10 panels, a 150mg sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. In cases where scalp hair is not obtainable, an equivalent amount of body hair may be used. The term head hair specifically refers to scalp hair. Body hair encompasses all other types like facial and axillary hair.

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into the laboratory's system. This step verifies that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and addresses any additional data entry required that is not available through an electronic chain of custody system.

Samples that are presumptively positive in Screening necessitate a secondary confirmation.

When a sample is presumptively positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are isolated from the hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of a positive screening result is conducted with GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as required. The complete laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
