What it is, How it works

A 1.5-inch sample of around 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp provides 100mg of hair, which is optimal for both screening and confirmation. For EtG add-ons and/or tests beyond 10 panels, a 150mg specimen is advisable. We recommend using a jeweler’s scale to weigh the specimen. If scalp hair isn't available, an equal portion of body hair can be taken. In reference to head hair, we mean only scalp hair. Body hair refers to all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial process of inputting a sample into a lab’s system. This step includes confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any other data entry not covered by an electronic chain of custody system.

Screening offers a preliminary test for substances of abuse. While it is a cost-efficient method to exclude drug use on most samples, a positive screen must be validated to be admissible in court. Any samples showing presumptive positivity in Screening need subsequent confirmation.

Should a sample indicate presumptive positivity during Screening, additional hair is taken from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a substantially reduced concentration compared to other techniques (e.g., urine or oral fluid), which explains why hair drug testing is the most complex technique to execute.

Confirmation of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The full laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
