For tests involving EtG, additional panels, and/or exceeding 10 panels, a sample size of 150mg is advisable. Utilizing a jeweler’s scale for weighing the sample is suggested. If scalp hair cannot be collected, an equivalent mass of body hair may be used. "Head hair" pertains specifically to scalp hair, while "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four principal stages in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a specimen into a lab’s framework. This involves confirming proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and filling out any additional data entry absent in an electronic chain of custody system.

Screening serves as an initial rapid assessment for drug abuse indicators. While this is a cost-efficient means to dismiss drug use for most samples, a positive screen must be confirmed for courtroom validity. Samples that are tentatively positive during Screening require further confirmation testing.

If tentative positivity is observed during Screening, additional hair from the original sample is extracted for preparation during the Extraction phase. During this phase, drugs are drawn from hair at significantly lower concentrations than through other methods (e.g., urine or oral fluids), making hair drug screening one of the most challenging methods to perform.

Confirmation of any presumptive positive outcomes from Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation as necessary. The entire lab process, from Accessioning to Confirmation, undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
