What it is, How it works

Hair analysis has become recognized as an influential method for detecting drug and alcohol consumption. Hair captures a prolonged history of substance use by embedding biomarkers within the strands of hair as they develop. When samples are taken near the scalp, hair can reflect a detection period of up to around three months for drugs and alcohol. The collection of hair is uncomplicated, somewhat resistant to tampering, and convenient to transport.

A hair sample measuring 1.5 inches consisting of approximately 200 strands (comparable to the thickness of a #2 pencil) near the scalp will provide 100mg of hair, which is an optimal amount for both screening and confirmation procedures. For EtG enhancements and/or tests exceeding 10 panels, a sample size of 150mg is suggested. We advise measuring the sample weight with a jeweler's scale. In case scalp hair is inaccessible, an equivalent quantity of body hair can be gathered. When referring to head hair, we specifically mean scalp hair. Body hair encompasses all other varieties of hair (such as facial or axillary hair).

Process Overview

The laboratory procedure for processing a drug test result pivots around four core stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial assimilation of a sample into a lab's system. This encompasses confirmation that the sample was appropriately sealed and dispatched, allocation of a random Laboratory Accessioning Number (LAN), as well as the completion of any supplementary data entry that isn't provided by an electronic chain-of-custody framework.

Screening constitutes an initial rapid assessment for substance abuse. While Screening serves as a cost-efficient means to exclude drug presence in the bulk of samples, positive screens require confirmation to hold up in legal settings. Any samples indicating a positive result in Screening necessitate additional verification.

If Screening suggests a positive result, additional hair is extracted from the initial sample for the Extraction phase. During this phase, substances are retrieved from the hair at lower concentrations than in alternative methods (such as urine or oral fluid), highlighting why hair drug testing is deemed the most challenging of methodologies.

Confirmation of any positive screens is performed via GC/MS, GC/MS/MS or LC/MS/MS. All samples indicating presumptive positives undergo washes as needed before confirmation. The complete lab procedure from Accessioning through Confirmation is subjected to scrutiny under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
