What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of substances by embedding biomarkers within the fibers of the hair as it grows. If collected near the scalp, hair can reveal drug and alcohol use for up to about three months. Collecting hair is straightforward, relatively hard to tamper with, and easy to transport.

A sample measuring 1.5 inches and consisting of around 200 strands (approximately the thickness of a #2 pencil) near the scalp yields 100mg of hair, which is the optimal size for both screening and confirmation purposes. For tests involving EtG, additional compounds, and/or panels exceeding 10, 150mg of the sample is advised. We suggest utilizing a jeweler’s scale for weighing. Should scalp hair not be available, equivalent body hair can be used. References to head hair pertain to scalp hair alone. Body hair encompasses all other hair categories (such as facial or axillary).

Process Overview

The laboratory process of analyzing a drug test result comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory’s system. This involves ensuring that the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing additional data entries not already included by an electronic chain of custody.

Screening consists of an initial cursory examination for drug abuse. Although Screening is cost-effective for excluding drug use in most instances, a positive screen must be verified for legal admissibility. Any samples that test presumptive positive need further confirmation.

For samples testing presumptive positive during Screening, additional hair is retrieved from the original sample and readied for Extraction. Here, drugs are extracted from hair at significantly lower concentrations compared to methods like urine or oral fluid testing, making hair drug screening particularly challenging.

Confirmation of any positive screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives undergo washing before confirmation as necessary. Each step of the laboratory process from Accessioning to Confirmation is conducted under the oversight of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
