What it is, How it works

When acquired close to the scalp, hair can provide a detection range of up to 3 months for alcohol and drugs. Hair is easy to gather, challenging to alter, and convenient to transport.

A hair sample of 1.5 inches containing around 200 strands (comparable to a #2 pencil's thickness) nearest to the scalp yields 100mg of hair, which is the optimum sample for both analysis and verification. For EtG, add-ons, and tests above 10 panels, 150mg of the sample is advised. We suggest weighing the hair sample using a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be used. By head hair, we refer solely to scalp hair. Body hair includes all other types like facial and axillary hair.

Process Overview

The primary steps in laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial processing of a sample into the laboratory's system. This entails verifying that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in any extra data not available through an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. While Screening serves as a cost-efficient way to rule out drug use for most samples, any positive result must be confirmed to be legally accepted. Any samples testing presumptively positive during Screening require further confirmation.

If a sample tests presumptively positive at the Screening stage, additional hair is collected from the original sample and prepared for Extraction. Drugs are extracted from hair at significantly lower concentrations than in other methods like urine or oral fluid, making hair drug screening a more challenging technique to implement.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The complete laboratory procedure from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
