What it is, How it works

A 1.5-inch segment of around 200 hair strands (resembling the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for both screening and confirmation. For tests such as EtG, additional panels, or tests exceeding 10 panels, a specimen of 150mg is suggested. We advise using a jeweler’s scale to weigh the sample. If scalp hair is not available, a similar quantity of body hair can be collected. In terms of head hair, we specifically refer to scalp hair. Body hair pertains to all other varieties of hair (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result is comprised of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial integration of a sample into a laboratory’s workflow. This step guarantees that the sample was correctly sealed and delivered, assigns a random LAN (Laboratory Accessioning Number), and carries out any additional data entry that is not covered by an electronic chain of custody system.

Screening performs an initial rapid analysis for drugs of abuse. Although Screening is a cost-effective approach to dismiss drug usage in most samples, a positive screen must be confirmed to be submitted in legal proceedings. Any samples that show presumptive positive results in Screening must undergo additional confirmation.

When a sample gives presumptive positive results during the Screening phase, more hair is extracted from the initial specimen and readied for Extraction. In this stage, drugs are retrieved from hair at significantly lower concentrations than other methods (such as urine or oral fluid), which makes hair drug screening the most complex methodology to execute.

Any positive screening outcome is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results undergo washing prior to confirmation as necessary. The entire laboratory method from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards of accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
