For EtG, additional checks, and tests beyond 10 panels, 150mg of the material is advisable. Using a jeweler’s scale for measuring the sample is suggested. If scalp hair isn’t accessible, a corresponding amount of body hair can be gathered. “Head hair” denotes hair from the scalp, while “body hair” encompasses all other hair categories (facial, axillary, etc.).

Process Overview

Processing a drug test result in the lab involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample in the laboratory’s system. This process consists of verifying the sample’s secure sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and performing any necessary data entry that an electronic chain of custody system hasn’t provided.

Screening acts as an initial, swift check for drugs of abuse. Although Screening is a cost-effective method for excluding drug usage in most samples, a positive screen necessitates confirmation to be legally valid. Any samples with a presumptive positive result in Screening must go through a secondary verification.

If the Screening process indicates a presumptive positive, additional hair is taken from the initial sample for the Extraction phase. In Extraction, drugs are retrieved from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation of positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo washing before confirmation if necessary. The entire process, from Accessioning to Confirmation, is thoroughly examined under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
