A sample measuring 1.5 inches of around 200 hair strands (approximately the thickness of a #2 pencil) next to the scalp yields 100mg of hair, ideal for both screening and confirmation procedures. For EtG, additional tests, and assessments exceeding 10 panels, 150mg of the sample is advisable. It's suggested to measure the sample with a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair may be used. Head hair denotes scalp hair only, while body hair includes all other hair types like facial or axillary.

Process Overview

The laboratory processing of a drug test result involves four essential phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is integrated into a laboratory's system. This step confirms proper sample sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by electronic chain of custody systems.

Screening provides an initial rapid assessment for drug presence. While Screening efficiently rules out drug usage in most samples, a positive screen must be confirmed for legal admissibility. Any samples with presumptive positives during Screening undergo further confirmation.

For samples that test presumptively positive in Screening, additional hair is extracted from the original sample for the Extraction stage. Here, drugs are isolated from hair at significantly lower concentrations than in methods such as urine or oral fluid tests, highlighting hair drug screening's complexity.

Any positive screening result is confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptive positive are cleaned before confirmation if needed. The entire lab process from Accessioning through Confirmation is reviewed under the CAP (College of American Pathologists) Hair title and the ISO / IEC 17025 accreditation standards.
