Obtaining a 1.5-inch section consisting of around 200 hair strands (roughly the width of a #2 pencil) closest to the scalp yields 100mg of hair, which is ideal for screening and confirming results. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg is advised. We suggest using a jeweler's scale for weighing the sample. If scalp hair isn't available, a similar amount of body hair can be collected. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The drug testing laboratory processes samples through four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into a lab’s system. This entails verifying correct sealing and shipping of the sample, assigning a random Laboratory Accessioning Number (LAN), and completing any extra data entry not available via an electronic chain of custody system.

Screening is a quick preliminary check for drugs of abuse. Although Screening economically eliminates drug usage in most samples, a positive result must undergo confirmation for legal admissibility. Every sample that’s presumptively positive in Screening mandates a secondary confirmation.

If Screening indicates a presumptive positive result, additional hair is extracted from the initial sample and prepped for Extraction. During this phase, drugs are extracted from hair at notably lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug testing is exceptionally challenging.

Confirmation of a positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation as well as accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
