For testing categories such as EtG, enhancements, and/or more than 10-panel tests, 150mg of the hair is advised. Using a jeweler’s scale for weighing specimens is recommended. If scalp hair is not available, a comparable amount of body hair can be utilized. Here, head hair refers strictly to scalp hair. All other hair types (such as beard or underarm) are considered body hair.

Process Overview

The lab processing of a drug test result involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a specimen within a lab's workflow, ensuring proper sealing and transportation, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any additional data entry that an electronic custody chain hasn’t supplied.

Screening provides a rapid test for drug abuse. This stage helps efficiently exclude drug usage in most cases, though a verified result is mandatory for legal contexts. Any sample testing positive in the initial Screening demands a follow-up confirmation.

When a sample yields a positive Screening outcome, extra hair is retrieved from the original batch and prepared for Extraction. At this stage, substances are removed from hair at significantly lower concentrations compared to other test types (such as urine or oral samples), which renders hair drug screening notably complex.

Positive test results from Screening must undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples indicated as positive in Screening are cleansed as required before Confirmation. The entire lab process from Accessioning to Confirmation complies with CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
