If scalp hair isn't available, an equivalent amount of body hair can be used. Reference to head hair specifically indicates scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of the sample into the laboratory's system, which encompasses checking that the sample was properly sealed and dispatched, assigning a Laboratory Accessioning Number (LAN) randomly, and completing any requisite data entry absent from an electronic chain of custody system.

Screening is a preliminary examination for substance abuse. Although screening is a cost-efficient method to eliminate drug use in most samples, a positive screen requires confirmation to hold up in a legal setting. Any samples marked presumptively positive during Screening necessitate secondary confirmation.

Should a sample test presumptively positive during Screening, additional hair from the original sample is prepared for Extraction. In this phase, drugs are extracted at concentrations much lower than other methods (e.g., urine or oral fluid), illustrating why hair drug screening is the most complex methodology to execute.

The confirmation of any positive screen result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Samples that test presumptively positive undergo washing prior to confirmation if required. The entire laboratory process, from Accessioning through Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair standards and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
