Employers maintaining zero-tolerance policies find value in hair tests for their prolonged detection time—up to 90 days. While nearly foolproof, these tests are not as effective in identifying drug use within the past 5 days, where urine tests remain most efficient.

What it is, How it works

Hair testing has emerged as an effective method for detecting drug and alcohol use. Hair captures a long-term record of alcohol and drugs by holding biomarkers within the fibers of the growing strand. When taken near the scalp, hair offers up to a 3-month period for detecting alcohol and other drugs. It's easy to collect, somewhat resistant to tampering, and convenient to ship.

A 1.5-inch portion of around 200 hair strands (comparable to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect amount for screening and validation. For EtG, additional panels, and/or tests exceeding 10 panels, 150mg of the sample is preferred. We advise using a jeweler’s scale for weighing the specimen. If scalp hair is not available, an equivalent quantity of body hair can be gathered. When mentioning head hair, we mean scalp hair only. Body hair includes all other hair types (such as facial, axillary, etc.).

Process Overview

The four key procedures in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s system. It ensures that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry that an electronic chain of custody system doesn't provide.

Screening is a preliminary, quick examination for drugs of abuse. Although Screening is a cost-efficient method to negate drug use in most samples, a positive screen requires confirmation for legal admissibility. Any samples that are presumably positive in Screening need further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample and processed for Extraction. In this stage, drugs are extracted from hair at a considerably lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug testing the most challenging methodology to conduct.

Confirmation of any positive screen is done via GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are washed before confirmation as required. The full lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
