What it is, How it works

Hair serves as a long-lasting record of alcohol and drug presence by capturing biomarkers within the strands as they grow. Collected near the scalp, hair offers a period of up to three months for detecting alcohol and drugs. Hair is easy to gather, hard to tamper with, and straightforward to transport.

A 1.5-inch section of roughly 200 hair strands (similar to the size of a #2 pencil) near the scalp provides 100mg, an ideal quantity for screening and verification. For EtG, additional requests, and/or tests beyond 10 panels, 150mg of the sample is preferred. We advise using a jeweler’s scale to measure the sample weight. If scalp hair is absent, an equivalent quantity of body hair can be used. "Head hair" specifically means scalp hair, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The key stages in laboratory analysis of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample within a lab's framework. This step ensures the sample's proper sealing and shipment, assigns a unique LAN (Laboratory Accessioning Number), and completes any manual data entry not automated by an electronic chain of custody system.

Screening conducts an initial assessment for drugs of abuse. While Screening effectively dismisses drug use in most samples, any positive findings require confirmation for legal acceptability. Samples speculated to be positive during Screening must undergo a secondary verification.

If Screening yields a presumptively positive result, additional hair from the primary sample is utilized for Extraction. During this phase, drugs are drawn from hair at much lower concentrations compared to other methods (e.g., urine or oral fluids), making hair drug testing the most complex to execute.

Confirmation of positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples deemed presumptively positive are cleaned before confirmation as necessary. The full lab process, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair guidelines and complies with ISO / IEC 17025 standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
