Hair collection is straightforward, relatively resistant to tampering, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to the size of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal amount for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler's scale to weigh the sample. If scalp hair is scarce, a similar quantity of body hair can be gathered. By head hair, we specifically mean scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial phase of processing a sample into a lab’s system. This step ensures that the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry absent from an electronic chain of custody system.

Screening provides an initial assessment for any drugs of abuse. Although Screening offers a cost-effective method to eliminate drug use presence for most samples, a positive screen must be confirmed to gain legal acceptance. Presumptively positive samples identified during Screening require a secondary confirmation.

Should a sample show presumptive positivity in Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at a lower concentration than other methods (e.g., urine or oral fluid), which underscores the challenge of performing hair drug screening.

Confirmation of positive screening results is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire laboratory process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
