For EtG, additional tests, and/or assessments beyond 10 panels, 150mg is suggested. Utilizing a jeweler’s scale to weigh the specimen is advised. In cases where scalp hair is not available, a comparable amount of body hair can be used. 'Head hair' specifically pertains to scalp hair, while 'body hair' refers to any other hair types (such as facial, underarm, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary integration of a sample into the lab's database. This step ensures that the sample was properly sealed and dispatched, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening serves as an initial rapid assessment for drug misuse. While cost-efficient for ruling out drug usage in most samples, a positive screen demands further validation for legal acceptance. Samples that are tentatively positive in Screening necessitate additional confirmation.

Should a sample test positive during the Screening phase, additional hair is extracted and prepped for Extraction. Drugs are withdrawn from hair at substantially lower concentrations than other methodologies (e.g., urine or oral fluid), making hair drug screening exceptionally complex.

Confirmation for any positive screening results takes place using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirmation, all provisional positive samples undergo cleaning. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
