Hair analysis has gained recognition as an effective method for identifying substance use involving drugs and alcohol. Hair retains a historical record of drug and alcohol consumption by capturing biomarkers within the strands as they grow. When sourced near the scalp, hair can provide a detection period of up to approximately three months for both alcohol and drugs. Collecting hair is straightforward, challenging to tamper with, and easy to transport.
A 1.5-inch hair sample consisting of around 200 strands (approximately the diameter of a #2 pencil) taken closest to the scalp will yield 100mg of hair, which is the optimal sample size for both initial testing and verification. For EtG, additional tests, and/or more than 10-panel tests, a sample weighing 150mg is advisable. We suggest using a jeweler’s scale for accurate measurement. If scalp hair is unavailable, a similar quantity of body hair may be used. When referencing head hair, we imply scalp hair exclusively. Body hair pertains to every other hair type (such as facial and axillary hair).
Process Overview
The core stages in the lab analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves the initial integration of the sample into the lab’s system. This step ensures the sample was correctly sealed and transported, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry absent from an electronic chain of custody system.
Screening includes a preliminary assessment for illicit drug use. While Screening is a budget-friendly approach to rule out drug consumption in most samples, a positive result requires confirmation to be valid in legal settings. Any samples that test presumptively positive in Screening require further confirmation.
Should a sample test presumptively positive in the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. During this stage, drugs are extracted from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug analysis is particularly challenging to conduct.
Confirmation of a positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples suspected to be positive undergo necessary washing before confirmation. The entire lab process from Accessioning to Confirmation is inspected under the guidelines of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

Note: Despite being commonly called "hair follicle tests", the actual analysis is done on the hair strand and not the follicle beneath the scalp.
