A sample of 1.5 inches, comprising around 200 hair strands (equivalent in size to a #2 pencil) nearest to the scalp, yields 100mg—ideal for screening and confirming purposes. For EtG, additional tests, or more than 10 panels, 150mg is advised. We suggest using a jeweler’s scale for accuracy. If head hair isn't accessible, collecting a comparable amount of body hair is viable. When referencing head hair, it pertains only to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The drug test result processing in laboratories involves four sequential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes initially processing a sample into the lab's system, ensuring the sample was sealed and transported correctly, assigning it a random LAN (Laboratory Accessioning Number), and filling in any additional information not provided by an electronic chain of custody system.

Screening performs an initial drug-of-abuse check. Although cost-effective for negating drug use in most cases, a confirmed screen is essential for legal admissibility. Samples showing initial positive results in Screening necessitate further confirmation.

Should a Screening stage return positive, more hair is sourced from the original sample for Extraction. This phase involves extracting drugs from the hair at lower concentrations than other tests (e.g., urine or oral fluid), making hair drug screening particularly challenging.

GC/MS, GC/MS/MS, or LC/MS/MS confirms any positive screening outcomes. Presumptive positive samples receive a necessary wash before confirmation. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
