Process Overview

The procedure for processing a drug test outcome in the lab consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning addresses the preliminary incorporation of a sample into the lab’s records. This step ensures the sample was properly sealed and transported, assigns it a unique LAN (Laboratory Accessioning Number), and handles any data entry not captured by an electronic chain of custody system.

Screening comprises an expeditious review for drugs of interest. Despite being an economical approach to dismiss drug use in most samples, a positive result from Screening must be substantiated for legal scrutiny. Samples appearing positive in Screening require subsequent confirmation.

Should Screening indicate a presumptive positive, additional hair is extracted from the original batch and readied for Extraction. This phase isolates drugs from hair at much lower concentrations than other techniques (such as urine or oral fluid), underscoring hair drug testing as the most challenging methodology to execute.

Confirmation of any presumptive positive results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples marked as presumptive positive are also cleansed before confirmation, when necessary. The entire process, from Accessioning through Confirmation, undergoes evaluation according to the CAP (College of American Pathologists) Hair criteria and accreditation per ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
