A 1.5-inch section comprising roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimum amount for tests and confirmations. For EtG, additional, and tests exceeding 10 panels, 150mg of hair is advised. Utilizing a jeweler's scale for weighing the hair is suggested. If scalp hair isn’t available, a comparable amount of body hair can be utilized. When head hair is mentioned, it specifically means scalp hair only. Body hair includes other types such as facial and axillary hair.

Process Overview

The laboratory processing of a drug test result includes four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial entry of a sample into a lab’s system. This step ensures the sample’s proper sealing and dispatch, assigns it a random LAN (Laboratory Accessioning Number), and completes any missing data entry not covered by an electronic chain of custody system.

Screening performs an initial swift check for drug abuse. While Screening is an economical way to exclude drug usage on most samples, a positive screen must be confirmed to hold up in court. Samples preliminarily positive during Screening need further confirmation.

If Screening suggests a sample is positive, additional hair is taken from the original specimen and readied for Extraction. In this phase, drugs are dissolved from hair at much lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug testing the most intricate method.

Positive screen results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all preliminarily positive samples are washed as needed. The entire process from Accessioning to Confirmation undergoes review under CAP (College of American Pathologists) Hair designation and is accredited to ISO / IEC 17025 standards.
