Collecting a 1.5-inch segment of approximately 200 hair strands (around the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal amount for analysis. For EtG, supplementary tests, and/or over 10-panel tests, a specimen of 150mg is advised. It's best to weigh the sample using a precision scale. In the absence of scalp hair, an equivalent mass of body hair can be gathered. The term head hair refers exclusively to scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Procedure Summary

There are four core stages in laboratory processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially integrated into the lab's system. This process involves ensuring the sample's intact sealing and correct shipment, allocating a random Laboratory Accessioning Number (LAN), and completing any necessary data entry absent in electronic chain of custody systems.

Screening serves as a preliminary assessment for substance abuse. Although Screening is an economical method for excluding drug use in most samples, any samples with a positive result require confirmation for court worthiness. All samples preliminarily found positive in Screening necessitate additional confirmation.

Should a sample test positive in the Screening phase, more hair is extracted from the original sample for the Extraction phase. During Extraction, drugs are isolated from hair at lower concentrations than those in other methods (e.g., urine or saliva), making hair drug testing a complex procedure.

Any positive results from Screening are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a washing stage prior to confirmation if needed. The entire laboratory process, from Accessioning to Confirmation, is rigorously audited per both the College of American Pathologists (CAP) Hair designation and ISO/IEC 17025 standards.
