Process Overview

The four fundamental stages involved in the laboratory handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning constitutes the preliminary processing of a specimen into a laboratory's framework. This step includes verifying that the sample was correctly sealed and dispatched, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any further data processing not covered by an electronic chain of custody system.

Though Screening is an economical method to eliminate drug use in most specimens, any positive screen must be verified for judicial acceptability. Any samples exhibiting presumptive positive results during Screening require a follow-up confirmation.

If a sample is deemed presumptively positive during Screening, additional hair is extracted from the original specimen and readied for Extraction. In this phase, drugs are retrieved from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug analysis the most complex methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation if needed. The entire laboratory procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
