What it is, How it works

Hair testing has become a recognized means for identifying drug and alcohol consumption. Hair captures a prolonged timeline of alcohol and drug use by embedding biomarkers within the fibers of the growing hair shaft. When gathered near the scalp, hair can offer a detection period of up to about 3 months for alcohol and drugs. Hair is straightforward to collect, relatively hard to tamper with, and easy to transport.

A 1.5-inch segment consisting of about 200 strands of hair (roughly the thickness of a #2 pencil) from the scalp provides 100mg of hair, which is ideal for screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen on a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair can be collected. When mentioning head hair, we refer exclusively to scalp hair. Body hair refers to all other hair forms (facial, axillary, etc.).

Process Overview

The core steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the preliminary processing of a sample into the lab's system. This process involves confirming that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is an initial swift check for drugs of abuse. Screening is a cost-effective way to rule out drug usage in most samples; however, a positive screen necessitates confirmation to be accepted in court. Any samples that are presumptively positive in Screening require subsequent confirmation.

If a sample tests presumptively positive in Screening, additional hair is taken from the initial specimen and prepared for Extraction. In this step, drugs are extracted from hair at a lower concentration than other methodologies (such as urine or oral fluid), which makes hair drug screening the most challenging method to conduct.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed as needed before confirmation. The entire lab process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and compliancy with ISO / IEC 17025 standards.
