When testing for EtG, additional substances, and those exceeding 10 panels, a specimen amount of 150mg is advised. It's advisable to use a jeweler’s scale for measuring the specimen weight. In the absence of scalp hair, an equivalent quantity of body hair is acceptable. When mentioning head hair, we solely mean scalp hair. Conversely, body hair refers to all other hair categories (facial, axillary, etc.).

Process Overview

The procedure for laboratory drug testing encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab's framework. This encompasses confirming that the sample is sealed and properly dispatched, attributing a random LAN (Laboratory Accessioning Number), and fulfilling any supplementary data entry omitted by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for substances of abuse. While Screening is a budget-friendly method to exclude drug consumption in most cases, a positive result from Screening must be validated to hold up in legal settings. Any specimens suspected as positive in Screening necessitate additional confirmation.

If a specimen appears positive in the Screening phase, additional hair is sourced from the initial collection and readied for Extraction. During this phase, drugs are isolated from hair at a significantly reduced concentration compared to other techniques (e.g., urine or oral fluid), making hair drug assessment the most complex technique to execute.

Any positive results from screening undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive specimens are cleansed before confirmation as required. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair category and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
