What it is, How it works

For EtG testing, add-ons, and/or panels exceeding 10, 150mg of the sample is advisable. It is suggested to measure the specimen using a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair may be used. When discussing head hair, reference is only made to scalp hair. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the lab system. This process includes checking that the sample is sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in any additional data required beyond the electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient means to exclude drug use in most samples, any positive results need confirmation to be valid in legal proceedings. Any samples that test positive during Screening require further confirmation.

Should a sample test presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at a more diluted concentration than with other methods (e.g., urine or oral fluid), which makes hair drug testing the most challenging method to execute.

Confirmation for any positive Screening results is performed through GC/MS, GC/MS/MS or LC/MS/MS. All samples testing presumptively positive undergo cleansing prior to Confirmation as necessary. The complete lab procedure from Accessioning to Confirmation undergoes review according to both the CAP (College of American Pathologists) Hair designation and the accreditation of ISO / IEC 17025 standards.
