When hair is trimmed near the scalp, it can reveal up to a 3-month period for detecting alcohol and drugs. Hair collection is straightforward, hard to tamper with, and convenient for transportation.

A sample of 1.5 inches consisting of about 200 hair strands (approximately the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, the preferred sample amount for analysis. For EtG, additions, and/or examinations exceeding 10 panels, a 150mg sample is advised. It is suggested that the specimen be weighed on a jeweler's scale. If scalp hair is not available, an equivalent amount of body hair may be used. References to head hair pertain exclusively to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial entering of a sample into the laboratory's framework. This step ensures proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry not available via an electronic chain of custody system.

Screening conducts a primary rapid assessment for substances of abuse. Although Screening is a budget-friendly method to negate drug usage for most samples, a positive result demands further confirmation to hold legal merit. Presumptive positive Screening samples must undergo additional confirmation.

If Screening suggests a sample is likely positive, additional hair is extracted from the original specimen for the Extraction process. Drugs are withdrawn from hair at much lower concentrations than in alternative methods (such as urine or oral fluid), highlighting why hair drug screening presents greater challenges than other approaches.

The confirmation of any positive Screening outcome is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed accordingly. The complete laboratory process from Accessioning through to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
