What it is, How it works

When gathered near the scalp, it can reveal up to a 3-month history of alcohol and drugs. The collection process is easy, hard to tamper with, and convenient for transport.

A 1.5-inch segment, roughly 200 strands akin to a #2 pencil's thickness, nearest to the scalp yields 100mg of hair, ideal for initial tests and verification. For enhanced testing like EtG or panels beyond ten, 150mg is optimal. We suggest using a jeweler’s scale for weighing. If scalp hair isn’t an option, a comparable quantity of body hair can be used. Head hair specifically refers to scalp hair; body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four core steps in processing a lab drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts the sample's journey in a lab, ensuring it's sealed and shipped correctly, assigning a LAN (Laboratory Accessioning Number), and adding any data omitted from an electronic custody system.

Screening provides a quick drug presence check. Although cost-effective in ruling out drug use for most samples, positives in Screening require confirmation for court acceptance. Presumptive positives need further verification.

During Extraction, more hair from the initial sample is prepped if positive in Screening. Drugs are then isolated from hair at lower concentrations than other methods (e.g., urine, oral fluid), making hair screens notably complex.

To confirm positive screenings, GC/MS, GC/MS/MS, or LC/MS/MS technologies are used. Presumptive samples undergo necessary washes before confirmation. The full processing from Accessioning to Confirmation follows CAP (College of American Pathologists) regulations for Hair and ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
